It is probably the proxy failing to load the image in and resorting back to phasers unloaded image witch is a black square with a green border with a red line going though it

Yeah really sucked for PvP though sense you couldn’t see your hit box lol you could get all the way to samurai this was right after it was removed and coder went back to the previous update but didn’t add image back so you got this wonky thing lol
